Millions need care at home, but waitlists keep growing. Unmet needs become ER visits, hospitalizations, and nursing home admissions.
Our clinical care model is an answer to the healthcare workforce shortage.
Patient outcomes data is for home health patients and compares the 60 days prior to beginning care to each 60 day period after beginning care. Rate is number of patients with any number of incidents, not number of incidents. Data is based on caregiver- and clinician-reported data.
